We have entered our 3rd manufacture - it should be available again in a few months - the delay is that we are changing how we procure FPGAs. Sorry for the delay.

I would like to release an RCA board which just plugs into the octo. No cables, I have one prototyped. However I don't know if people will be interested in it.

Installed a new SD car with new OS, followed the instructions... to no avail...grrrrr... so being an ee, I went and looked at the schematics and found the answer... now I get stereo audio output :D Nowhere in the product description does it state how many inputs there are and how many outputs :o . ...

Yes - it looks like it is setup correctly.

I still have the feeling that these boot issues are to do with timings. My suspicion is that it is something to do with DC decoupling caps requiring a large charge time, or something similar. I Think you would have to create a little connector which breaks the GND and mic. line out from the headset and feeds that into the microphone connector.
Then you can break the stereo lines to the line outputs - headphone or other !
