I have put the PCBs for the breakouts up on github here :

You can fabricate them using the files in the gerbers directory.

Here is a setup fabrication you can use for a very low cost.

Do you have a parts list for the various connectors/components? (I know they are simple just figured I would ask)

It looks like the break out boards support multiple audio jack types?

Yes they support multiple different audio jacks.

Some of these potentiometers are a good fit :

You can buy break out boards in the nearby system accessories shop or you can buy it through online if you know the site is genuine. It does not cost much. Be careful when you deal with online dealers
