Help No Output

Hi, I have set this card up and installed as per instructions.

The Pi knows it is there and has a volume control, but there is no sound from either the headphones or the line outs.

I have been trying for roughly 7 hours now and have gave up, please can someone tell me what is the problem and the solution, thank you.
